If the TOC has been generated automatically, e.g. with MS Word and/or Adobe InDesign, based on their feature to turn the occurrence of text style levels into a proper Table of Content, then you're rewarded with a fully automated process of creating links right into the PDF. Just check the appropriate export options of your software.

If the TOC is just a bunch of typed or copied headings and a tediously collected and typed list of page numbers, then you're out of luck. You'll need to manually link these lines or words to the correct page. And again, when you edit any content...
