When you combine multiple PDFs into one by using the applicable command in e.g. Acrobat, the Bookmark panel will be populated with a clickable entry for each PDF. The text ill be the title (often the same as the file name) of the PDF, but you can easily edit them manually.

Be sure to chose the Pages + Bookmarks option in the Initial View panel of the Document Properties, so the viewer will open the file with the Bookmarks panel already displayed.
